Welcome Pharmacy Students
To familiarize yourself with the PharmD. Program’s application process, please use the Fall 2008 PH Guideline Booklet. Students interested in applying to the PharmD. Program should click the link to view and or print the guideline booklet. Potential students that have further questions are encouraged to contact the admissions office at 718-488-1011. Applicants must attach a copy of the Coursework in Progress Sheet to their application before sending the document to the Admission’s office. Applicants that choose to submit their application online should print and send a completed copy of their Coursework in Progress Sheet to the Office of Admissions. Please keep in mind that all educational documents from countries outside of the United States must be evaluated and translated by a reputable credit evaluation/translation firm. Please click on the Accredited Evaluators list to view the list of the acceptable evaluation agencies.
Key information:
* All students, including current LIU student, interested in applying to the professional phase of the PharmD. Program must apply through http://www.pharmcas.org/
* All international students that apply through PharmCAS.org with educational documents from countries outside of the United States must have their documents evaluated and translated by a reputable credit evaluation/translation firm. (See Accredited Evaluators list)
* All students, including current LIU students interested in applying to the professional phase of the PharmD. Program for Fall 2009
must take the PCAT no later than October 18, 2008 (http://www.pcatweb.info)
* Please keep in mind that the application deadline is January 5th, 2009 for the Fall 2009 semester.
