Dear Parent Borrower:

            The completion of this form is required by the parent who is applying for a Federal Direct PLUS Loan for the 2007-2008 award year. This loan will help meet your dependent's educational expenses at the Brooklyn Campus of Long Island University. After all the appropriate forms have been submitted to the Office of the Office of Student Financial Services, they will notify you of your PLUS loan eligibility.

            If there is a credit on the student's account resulting from the PLUS loan, the refund check will be made payable to you. Please note that the amount of the refund will be only for that portion of the credit resulting from the PLUS loan. Any difference will be issued to the student.

            I authorize the Brooklyn Campus of Long Island University to apply the PLUS loan to all tuition, fees, room, board and miscellaneous charges incurred by the above student during the award year indicated. I also authorize application of these funds to the student's prior outstanding balance, if any, which is reflected on the student's bill. I understand that this authorization will remain on file in the Office of the Bursar and that I may rescind this authorization prior to the disbursement of the funds.
