Immunization Requirements

Immunizations Alert

IMMUNITY TO MEASLES, MUMPS, RUBELLA, AND MENINGOCOCCAL VACCINES:

**Effective Fall 1990, Immunization required for all students born after January, 1957**
**Effective August 15, 2003, Public health Law #2167 requires students taking at least 4 hours per semester must be vaccinated against the Meningococcal Meningitis Vaccine or sign a waiver**

 


In order to comply with the New York State Immunization Public health Law 2165, you must fulfill one of the following categories.

1. DOSES REQUIRED OF LIVE VIRUS VACCINE:

Measles: Two doses at least 28 days apart, which must be after first birthday and after 1957
Mumps: One dose after 1969
Rubella: One Dose

OR

MMR: Two doses at least 28 days apart, which must be after first birthday and after 1972

 

OR

MMR: One dose after first birthday and after 1972
Measles: One Dose after first birthday and after 1972
*These two doses must be at least 28 days apart*

 

2. DOCUMENTATION OF MEDICALLY DIAGNOSTIC DISEASE:

For Measles and Mumps only!!! Not Rubella. Diagnosed disease of Rubella not accepted.
Dates must be shown: This includes month, day and year.
You must still show vaccine records for Rubella.

3. SEROLOGICAL EVIDENCE OF IMMUNITY FOR EACH DISEASE:

Blood Titers for Measles, Mumps and Rubella must show positive immunity.
Labs results must be attached.

  • Free inoculations are being offered at the New York State Department of health, 303 Nineth Ave Manhattan Chelsea clinic Immunization hotline  212 676- 2273 - Mondays and Fridays on a first come first serve basis
  • Please make copies of documentation before handing it in.
  • Health Services does not make or give copies of immunization records.
