Forms and Guides
for
Long Island University Brooklyn Campus
This page contains Electronic Reserves guides and all the forms you need
to create, update, and add documents to your Electronic Reserve
course page.
This form
must be filled out completely for every course you would like
to create an ERes page for.
This is a pdf form, which may be filled out in Acrobat
(click on the Name field to begin, then use the mouse
or tab key to move from field to field) and then printed, or printed
and filled out by hand.
Completed forms may be sent to the Circulation Department,
5th floor LLC by campus mail.
2. Course Reserve form for articles and packets (pdf)
This form must be filled
out completely, with complete bibliographic information for each
article, chapter, and document you would like to place on your
ERes course page. This form also may be filled out either
in Acrobat or by hand, and then sent to the Circulation department.
3. Student’s Guide to ERes (pdf)
You will
receive enough copies of this instructional guide for your class
from the Circulation department when your course page is created. You may print and photocopy extra copies
of this guide as needed.

to Copyright law and Electronic Reserves.
5. Copyright Permission Letter-Articles (Word)

This form letter is
used to request permission from the copyright holder of an article
to use that article on Electronic Reserves.
Permission must be obtained for any article used for more
than one semester, or when more than one article from the same
volume and issue of the a journal is used during one semester. This form may be filled out in either
Word or Acrobat, printed on LIU letterhead, and sent to the publisher/rights
holder of a work. Once
the form has been signed by the publisher/rights holder it must
be submitted to the Circulation desk.

8. Copyright Permission Letter-Chapters (pdf)
This form letter is
used to request permission from the copyright holder of a chapter
to use that chapter on Electronic Reserves.
Permission must be obtained for any chapter used for more
than one semester, or when more than one chapter from the same
book is used during one semester.
This form may be filled out in either Word or Acrobat,
printed on LIU letterhead, and sent to the publisher/rights holder
of a work. Once the
form has been signed by the publisher/rights holder it must be
submitted to the Circulation desk.
